Congratulations! Your laying hens, meat chickens, and breeding geese have been conditionally certified by the Animal Welfare Approved program. The final step is to sign and date the attached affidavits and return them to me as soon as possible. We require a physical signature on the affidavits. Please sign and date the attached and return them to me by scanned email, fax to 202-446-2151, or by mail to the address below. Once I have received them, I will send you an AWA logo, approval package, and certificate.

Please note that your pigs and turkeys will need to have the slaughter process reviewed before we can certify those species.
